Why does my dog use the bathroom relapse?

I have an 10 month old male shih-tzu named Bentley. It is a great dog, and was domesticated by the age of 3 or 4 months. He relieves himself in a urinal "pillow" that remains on the back porch screening. It has recently begun a problem which we consider very serious. Instead of going to the door so we let him out, he decided that he just pee on the couch or in bed. He is crate trained, so I left him in his cage to close all the time is not a problem, but he has been very good for so long that we can not understand relapse (and why he just want to pee on the couch and our bed). He does not sleep with us, except for "special occasions" (Usually when my husband is out of the city), but why not pee on the floor? We have a loft bed, so it's a big leap to make just to pee there. He has done several times in the past month or so, and the same goes for the couch too. Does anyone have any advice for our breeding semi-relapse?
